Suncrest Hospice - - Responsibilities: Provide resident care to assigned residents in accordance with resident assessments, care plans and as directed by nursing management; Coordinate the proper types of care with licensed care staff; Follow all nursing policies and safety guidelines; Collaborate with Case Manager/DON and patient care team; Be willing to drive to patient locations as needed
